Expressions of Interest for Social Prescribing Service in Laois and Meath

The Head of Service for Health and Wellbeing, HSE Midlands Louth Meath CHO (CHO8) – (Laois / Meath), is inviting expressions of interest from organisations to deliver a Social Prescribing Service in 1 * Community Healthcare Network Area in Meath and in 1 * Community Healthcare Network Area in Laois by employing and managing a Social Prescribing Link Worker post in each area. The Social Prescribing Service will operate across 1 Community Health Networks (CHNs) in the Laois area and 1 CHN in the Meath area.

Successful organisations will be contracted by HSE via Section 39 Grant Agreement (Health Act 2004) to deliver a specified service.
